Preventative Plan ($395 Annually)

  • 1 comprehensive exam with X-rays
  • 1 six month check-up with bitewing X-rays
  • 1 Emergency visit per year with single tooth X-ray
  • 2 annual basic prophy cleanings (Only for patients with healthy gums)
  • 2 Fluoride treatments
  • 20% off deep cleaning (for patients with gum disease and heavy tartar build-up)
  • Includes everything in the Preventative plan
  • 1 annual custom night guard or at home custom whitening trays
  • 15% off all other dental services provided in house
  • Includes everything in the Wellness plan
  • 20% off all other dental services provided in house
  • 20% off Endodontic (root canal) services with affiliated specialty practice
  • 1 comprehensive exam with X-rays

Membership plans are for patients without existing dental insurance. They cannot be combined with any other PPO/Medicaid dental insurance plans. Membership plans are non refundable/transferable and must be purchased in full. Membership is valid for one year from date of purchase.

Financing Your Care


In our office, we offer high-quality care and payment options that work for all of our patients. Below you will find a list payment options that we accept. Payment options:

  • Cash: Always accepted.
  • Credit Card: Visa, MasterCard, Discover, or American Express.
  • CareCredit: Apply online (www.carecredit.com) or in our office.

Please note: Payment is due at the time of service.

As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
